Roles and Responsibilities in Permit Confined Space Entry

Everyone involved in a confined space entry has a defined role — and each role is critical to safety.

Why It Matters:

Confined space incidents often happen because someone didn’t understand their responsibilities or assumed someone else had it covered. Clear roles ensure hazards are controlled, communication stays open, and help is ready if something goes wrong.

Key Points:

  • Entrants must know the hazards, wear required PPE, and follow instructions inside the space.
  • Attendants must stay outside the space, keep in constant communication, and never leave their post.
  • Entry supervisors are responsible for verifying conditions, ensuring permits are accurate, and authorizing entry.
  • No one should enter a space unless all roles are filled and the entry plan is understood.
  • If any role is compromised (someone leaves, loses contact, or is unsure), the entry must stop.
