Guard or Cover All Floor Openings

Unprotected holes are a serious hazard — guard them or cover them, no exceptions.

Why It Matters:

Falls through floor openings can lead to severe injury or death. Even a small hole can trip someone or catch a boot, especially if it’s poorly marked or uncovered. OSHA requires that any floor opening be either securely covered or surrounded by a guardrail system.

Key Points:

  • Cover all holes 2 inches or larger with material that can support twice the expected load.
  • Secure covers to prevent movement, and label them “Hole” or “Cover.”
  • Use guardrails around larger openings if covering is not practical.
  • Do not remove covers without proper authorization and re-cover immediately after.

✅ If there’s a hole, there should be a cover or a guardrail.

Ask the Crew:

  • Are all floor openings on site currently protected?
  • Have we reported any missing covers or unguarded holes?
