Toolbox Talk:

Bolts, Pins, and Latches Must Be Secure

June 22, 2025

The smallest part can cause the biggest failure. If a bolt backs out, a latch sticks open, or a cotter pin breaks — your entire lift is compromised.

Why It Matters:

Loose or missing fasteners, pins, or latches are one of the most overlooked causes of rigging failure. These components keep hooks closed, shackles secured, and connections intact. When they fail, loads fall — fast.

Key Points:

  • Always check that pins and bolts are tight and in place. Use locking mechanisms where required.
  • Make sure safety latches work freely. A jammed or broken latch turns a hook into a drop hazard.
  • Use correct hardware. Don't mix pins from different-sized shackles or substitute missing parts.
  • Watch for wear around pinholes and threads. Elongated holes or stripped threads reduce holding strength.
  • Tag out any rigging with damaged or missing components. Never improvise with tape, wire, or nails.

Secure hardware = secure lift.

Ask the Crew:

  • Do we check pins and latches before every lift?
  • Are we using the right components for the job?
  • Have we tagged out any gear with missing parts?
